This decision was made thoughtfully, and our top priority now is making sure every patient continues to receive the compassionate, high-quality care you deserve. To schedule an appointment, please call (909)706-7910.<\/p>\n<p class=\"p1\">If you need assistance with locating a new oral healthcare provider closer to Rancho Mirage, please contact us at (760) 565-6055.<\/p>\n<p class=\"p1\"><strong>Requesting Your Dental Records\u00a0<\/strong><\/p>\n<p class=\"p1\">You have the right to a copy of your dental records. Simply complete and sign our Medical Record Request and Consent Form, indicating whether you&#8217;d like your records sent to you directly or forwarded to your new dental provider. You can:<\/p>\n<ul class=\"ul1\">\n<li class=\"li1\">Download the Medical Record Request and Consent Form located on this page<\/li>\n<li class=\"li1\">Request the form by contacting our offices at (760) 565-6055 and one can be sent to you electronically via email using the email address we have for you on record.<\/li>\n<\/ul>\n<p class=\"p1\">Once we receive your signed form, we will take care of the rest.<\/p>\n<p class=\"p1\">After the We Care closes, your dental records will continue to be securely maintained in accordance with all applicable federal and state laws.
