We would like to inform our patients that Western U Health Oliver Station will be permanently closing as of August 28, 2026.
We are not accepting any new patients.
Our last day of seeing current patients in the clinic will be June 30, 2026.

Medical Records:
It is your right to have a copy of your medical record.
You may request a copy of your medical records by contacting us at 503.771.0055 until June 30, 2026. After June 30, 2026, please request your medical records by emailing medicalrecords@westernu.edu.

Please fill out the form, sign it, and return the completed form to the WesternU Health Medical Records office.
We will mail it to the address you provide or you can pick up your records from our office.
