If you do not know which doctor to see, go to Find a Provider for help.

Find a Provider

Finding a doctor at WesternU Health is easy. You may search according to your health condition or specialty area.

If you prefer, you can go to the Directory of Doctors and choose someone from the list.

If you’re not sure which service you need, look for a Primary Medical doctor.

Language Assistance

We take communication between you and your doctor very seriously. We want you to be able to understand and be understood by your doctor.

If you are non-English speaking or have limited English skills, we can help you talk with your doctor.

If you have other deficits such as a hearing or speech impairment, our team of translators and interpreters are here to help you understand and communicate with your doctor.

Just fill out the information and submit it online. Or, print it and bring the completed form with you to your first appointment.

For the Travel Health appointments, please complete Travel Health Questionnaire and bring it to your appointment.

It is important for us to know your medical condition and where you are planning to travel so that we can give you the proper immunizations and recommendations.

Request Your Medical Record

It is your right to have a copy of your medical record.

To request a copy of your medical record, download the Medical Records Request Form.

Just fill out the form, sign it, and return the completed form to the WesternU Health Medical Records office.

We will mail it to the address you provide.

Or, you can pick up your records from our offices.
